The ultimate goal is to support the systematic adoption, implementation, and evaluation of successful programs, practices, and policy changes. The database provides carefully reviewed, documented, and ranked practices that range from good ideas to evidence-based practices.

Goal: The goal of this project is primarily to empower patients who may be utilizing 911 or the ED for issues that can be better managed either at home or by a primary care provider by providing social support and strategies to reduce potential and/or perceived barriers to improved health.
Impact: Patients have benefited from the social aspects of the program becoming more active in the community, be it taking the bus, going for walks or visiting local food pantries along with an increase in patient compliance and self-reported increase in quality of life.

Goal: Tri 4 Schools encourages kids to participate in swimming, biking, and running as a way to live an active lifestyle and support health and fitness resources in schools.
Impact: Results of Exercise to Achievement are pending completion of the pilot program.

Goal: HeartStrings aims to enhance the quality of life for individuals with special needs. Through regularly-recurring, interactive sessions, the MSO’s Rhapsodie Quartet addresses the physical, emotional, cognitive, and social needs of hundreds of individuals each year in a comfortable and familiar setting.
HeartStrings mission statement is as follows:
1) To enhance the quality of life of underserved populations through live, interactive, and exceptional quality musical experiences that are informed by the American Music Therapy Association’s Standards of Practice.
2) To bring meaningful arts experiences directly to participants in a comfortable and
familiar setting.
3) To provide a valuable resource for facilities that serve aging populations, adults with dementia, and individuals with disabilities, or long-term illnesses.
Impact: HeartStrings has reached over 3,200 individuals with disabilities, long-term illness, and assisted-living needs, as well as aging adults with dementia since its start in 2006.

Goal: The goal of the Sustainable Skylines Initiative (SSI) is to achieve measurable emissions reductions and promote sustainability in urban environments within three years of implementation.

Goal: The Healthy Lifestyles Initiative works to integrate efforts to increase the proportion of Kansas City children and their families practicing healthy behaviors at a healthy weight.
Impact: Collaboration among multiple agencies across public and private sectors resulted in reaching almost 1 million people in the Kansas City area. Healthy weight assessments and plans received at primary care clinics are helping to promote healthy eating and living among children and their families.

Goal: As a leader in Kansas City's emergency food network, ECS is committed to providing access to healthy food for the community's food-insecure households. Its mission is to engage the Episcopal and broader communities in feeding the hungry and empowering the poor to move beyond the barriers of poverty with dignity - in short, feeding the hungry & changing lives. ECS is best known for the Kansas City Community Kitchen (KCCK) in the heart of the urban food desert. ECS also works to provide meaningful training experience through the Culinary Cornerstones Training Program, a 30-week immersive program preparing individuals for careers in the culinary world.
Impact: Since implementing the new service model, there has been a 10% average increase in the number of daily meals served at the Kansas City Community Kitchen. There has also been a large increase of volunteers, with an increasing number of recurring volunteers.

Goal: Shepherd's Center Central is a local leader in the regional effort to ensure that all people can age successfully with dignity, security, and respect.
Impact: In 2016, Adventures in Learning experienced attendance of 80 members per week and offered a total of 520 classes during the four 10-week sessions available.

Impact: The Medical-Legal Partnership provides legal assistance to patients on issues that directly or indirectly affect health.

Goal: ReStore Kansas City was created to be an entrepreneurial, fundraising arm for Habitat for Humanity Kansas City through sales of donated new and used building materials, furniture and appliances. By salvaging useable materials from people who no longer need them, ReStore is saving space in landfills, giving the public affordable materials, and helping to further the mission of affordable housing in the Kansas City area.
Impact: Two stores raised more than $5 million and approximately 60,000,000 lbs. of material were diverted from going into the local landfill.
