The mission of the Healthy Kids Express Asthma Program is to provide quality asthma care and education to children with asthma and their caretakers in a school environment to work towards goals set by Healthy People 2020 and the National Asthma Education and Prevention Program (NAEPP).
The Healthy Kids Express Asthma Program serves approximately 600 children a year, improving inhaler technique and asthma knowledge among participants. Children enrolled in the program for two consecutive years have lower school absentee rates and hospitalizations due to asthma.

The Pennsylvania Department of Aging offers this fall risk screening and prevention program to adults 50 years of age and older. The program is designed to raise awareness of falls, introduce steps on how to reduce falls, improve overall health, and provide referrals and resources.

Implementation of the program resulted in a decrease in addition severity, a decrease in risk behavior, and significant improvement in behavioral skills and quality of life.

The goal of this experiment is to estimate the effects of New York’s plan for supportive housing for high-need, high-cost Medicaid recipients.
Placing people who are homeless in supportive affordable housing paired with supportive services such as on-site case management and referrals to community-based services can lead to improved health, reduced hospital use, and decreased health care costs.

Immunization case management increased the knowledge of immunization schedules, rate of well-child visits, and up-to-date immunization rate for children of participating families.

The goal of this program is to increase provider recommendation and patient compliance with colorectal cancer screening at a federally qualified health center serving low-income patients.
The intervention appears to be a feasible means to improve colorectal cancer screening rates among patients served by community health centers. However, more attention to patient decision making and education may be needed to further increase screening rates.
